Requirements

• 1.Schengen Visa Application Form (duly completed and signed)
• 2.Passport valid for at least three months beyond the intended date of departure from the Schengen area, with at least two blank pages, issued within the last 10 years
• 3.Two recent passport-sized photos (3.5 x 4.5 cm), in color, against a white background
• 4.Proof of accommodation (hotel booking, invitation letter from host in Slovakia)
• 5.Round-trip flight reservation or itinerary
• 6.Travel medical insurance covering emergency medical expenses, hospitalization, and repatriation, with a minimum coverage of 30,000 EUR, valid throughout the Schengen area
• 7.Proof of sufficient financial means to cover the stay (bank statements for the last three months, salary slips, sponsorship letter)
• 8.Employment certificate (if employed) or business registration (if self-employed) or student certificate (if student)
• 9.Letter of invitation (if visiting friends/family) or official invitation from a company (for business trips)
• 10.Original birth certificate and marriage certificate (if applicable)
• 11.Proof of civil status (e.g., marriage certificate, birth certificate of children, death certificate of spouse, if applicable)
• 12.For minors: consent from both parents/guardians, along with their passports/ID cards
