Requirements

• 1.Valid passport (valid for at least 3 months after the declared departure date from Ukraine, with at least two blank pages)
• 2.Visa application form (completed and signed)
• 3.One recent passport-sized photo (3.5x4.5 cm)
• 4.Proof of sufficient funds (e.g., bank statements, credit card statements, sponsor letter) for the duration of stay (approx. $750 for 30 days, or $50 per day)
• 5.Travel medical insurance (covering the entire period of stay, with a minimum coverage of 30,000 EUR)
• 6.Proof of accommodation (hotel booking, invitation letter from host)
• 7.Confirmed round-trip tickets or itinerary
• 8.Purpose of visit documentation (e.g., invitation letter from Ukrainian entity/individual, tourist voucher, business invitation, medical treatment confirmation, student enrollment letter)
• 9.Consular fee payment receipt
• 10.Original birth certificate (for minors)
• 11.Parental consent (for minors traveling alone or with one parent)
